A Home Medicine Review allows an accredited pharmacist and physician to do a "check up" on a patients medications. It also provides an opportunity for patients to ask a clinical pharmacist questions or raise concerns in relations to their current medication regimen.

Key Stages of a Home Medicines Review
1. Doctor completes a HMR referral form
A specialist physician or general practitioner will decide together with a patient, if a HMR will assist them with better management of their medicines. A HMR referral form can be downloaded and submitted via the "contact" page on this website.

2. Accredited pharmacist will meet with the patient to perform a Home Medicines Review
An accredited pharmacist will contact the patient and arrange for a HMR interview to be completed, preferably in the patients home or via Telehealth.

3. Accredited Pharmacist will assess the patients medicines and submit a HMR Report to the referring physician
The accredited pharmacist will thoroughly asses the patients medications and information attained during the interview, and then submit a recommendation report to the referring doctor for their review.

4. Doctor and patient will discuss the pharmacists recommendations and alter medications if required
Any medication changes will be discussed with the patient and/or family and communicated to the pharmacist supplying the patients medications. For every initial comprehensive medicine review, a further two follow-up reviews can occur. This would usually occur one to three months after the first review.
